Hospitals are replacing legacy imaging systems and moving to the cloud. The replacement decision is really a data decision.

A wave of imaging system replacements is underway. Health systems of every size are moving off legacy PACS, and many are moving to the cloud at the same time. If you are facing a renewal or a replacement, the most important thing to understand is that you are not really buying a viewer. You are deciding who controls your imaging data for the next decade.

The cost that a sticker price hides

When imaging lives inside a proprietary archive, the number that hurts is rarely the license fee. It is the cost of getting your data back out. Egress charges, retrieval fees, and interface work can sit quietly inside a bundled price and only surface when you try to leave, or when you try to connect something new. By then you have little leverage. A modern imaging decision should price that exposure up front.

What to demand

Whatever system you choose, hold it to a short, non-negotiable list built around your ability to keep and move your own data:

  • Open standards, in practice. DICOM support across both DIMSE and DICOMweb (WADO-RS, QIDO-RS, WADO-URI), a configurable HL7 engine for orders and results, and PIX and PDQ patient matching. Ask for the conformance details, not the brochure.
  • Documented, fee-transparent export and migration terms in the contract, so a future move is a known quantity rather than a hostage negotiation.
  • No proprietary lock on your data. Your studies should be retrievable in standard formats without a custom project every time.
  • Portability as a first principle, so adopting a new viewer, a new site, or a new AI tool does not require rebuilding your foundation.

Why this framing wins

A neutral, portable archive changes the balance of power in your favor. It lowers your true total cost of ownership by removing the data-exit tax that proprietary systems rely on. It protects your ability to negotiate, because the incumbent knows you can leave. And it future-proofs you against the next shift, whether that is a cloud move, a network mandate, or a new clinical need.

The takeaway

The best imaging system decision is the one that protects your ability to make the next one. Demand data independence now, and the replacement you make today does not become the lock-in you fight tomorrow.
